Indications and optimal timing for surgery in infective endocarditis.

چکیده
M ore and more patients are operated on during the active phase of infective endocarditis (IE). Between 1991 and 1999, in France, the rate of cardiac surgery has increased from 31% to 50%. This may, at least in part, explain the concomitant decrease of in hospital mortality, from 22% to 17%. Nonetheless, the indications for cardiac surgery and its optimal timing remain difficult decisions. The principal indications for cardiac surgery are heart failure, no control of infection, embolisms, large size of vegetations, severe valvar and perivalvar lesions, and infection caused by some microorganisms.

متن کاملInter-Operative Time between Neurosurgery and Cardiac Surgery for Native Valve Endocarditis
Introduction: Infective endocarditis is well known to produce neurologic complications from septic emboli, which mandates a multidisciplinary approach between neurosurgery and cardiac surgery. However, literature is not clear as to the optimal interval length between neurosurgery and cardiac surgery.Presentation of Cases: The first case involves a 60-year-old woman with significant past medical...
متن کاملNeurological risk in urgent valve replacement cardiac surgery for infective endocarditis with cerebrovascular complications - a case series
Background/Introduction The indications for urgent valve replacement in patients with infective endocarditis (IE) include uncontrolled infection, recurrent emboli, valve dysfunction and cardiac failure. The optimal timing of this surgery when IE is complicated by cerebrovascular events (such as cerebral infarction or haemorrhage) is controversial, largely due to the escalated risk of perioperat...

Two factors changed the clinical course of infective endocarditis dramatically: 1) The discovery and evolution of techniques for identifying and treating its microbiologic causes and 2) Valvular surgery. We retrospectively evaluated 43 (33.5%) patients (8 female, 35 male) from 4 to 65 years old of 128 patients with infective endocarditis who underwent surgical intervention. Indication for surge...
